This patient consulted with Dr. William Vinyard about immediate breast reconstruction options after her planned bilateral mastectomies. They had a thorough discussion regarding the various breast reconstruction options and the associated risks, benefits, expectations, limitations and alternatives to each option. Dr. Vinyard performed a detailed history and physical examination where he identified her specific anatomic variations and asymmetries that would influence the breast reconstruction outcome. They both decided that immediate bilateral breast reconstruction was a good option for her. Dr. Vinyard created a custom surgical plan for her that included Bilateral Nipple Sparring Mastectomies followed by Bilateral Immediate Implant-Based Breast Reconstruction to create a more aesthetic and natural appearance. The implant-based breast reconstruction is a two step process where a temporary implant designed to create a natural breast shape and is adjustable to the patient's ultimate desired breast size is placed, along with an internal acellular dermal matrix for support, at the time of her bilateral mastectomies. During the second procedure, which is performed in an outpatient setting, the temporary implant is replaced with a softer long-term breast implant that is designed for comfort and to conform to the new breast shape that was created by the temporary implant.
